Please log in to our Patient Portal prior to your visit to complete necessary paperwork and Check in for your visit. You will need a working email address and a link will be sent to you. You may access the portal here: Health Portal

The paperwork can also be filled out in clinic, but you may need to come in a little earlier than scheduled to give yourself adequate time. Another option is to print and fill out the New Patient Forms prior to visit and bring them with you.

When you come for your appointment please remember to obtain and bring the following:

• Driver’s License or a valid ID

• Current Insurance Cards

• Referral Letter (if required)

• Disc of images with associated reports of any X-rays, MRIs, CT scans etc. and any other relevant information you may have obtained prior

• List of medications (if any)
